WHAT IS HOSPICE ?

When curing a serious illness is no longer possible, many people choose to stop curative treatment and accept palliative care. Palliative care means emphasis is placed on comfort and control of symptoms. It is not intended to prolong life. Hospice Care is provided primarily in the home, nursing home, or in an assisted living facility.

The Hospice Team assists family members in caring for their loved one in the comfortable and familiar surroundings of their own home whenever possible. Anova Hospice not only focuses on the special needs of the patient, but also provides support to their families. With our team of medical professionals, Anova Hospice strives to meet the physical, emotional, social, and spiritual needs of both the family and the patient.

With the expert guidance and counseling of our Hospice Team, Patient and families can make their own decisions about what is important to them. BENEFITS OF HOSPICE 

Who is eligible to receive hospice care?

 If you have Medicare you can use your home health care benefit if you meet all of the following conditions: 

Your doctor must order the home health care you are 'to receive.You must need one of Me following: 

intennittent skilled nursing care. or Ormeal/Speech Mae. or srroach-language Mara.. or comma. to nsai occupational therapy

You must be homebound, or normal, unaNe to leave you, home unasasted 

To be Homebound means that leaving home takes considerable and taxing effort

A person may leave home for medical treatment or short infrequent non-medical absences such as a religious service or a trip to the barber

The home health agency must be Medicare-certified

Palliative Care in Western Pennsylvania 

Mission:

If you have a patient that is homebound or who you think may qualify for home care under any of the criteria listed, Anova Health Care System will complete an initial in-home physical assessment and evaluation. Our specialty-trained registered nurses will complete an extensive physical evaluation; obtain lab work, if needed and our staff will then relay all the necessary facts to assist the physician in making those important decisions.

HOW DO HOSPICE SERVICES START?

Help is just a phone call away. Anyone can call Anova Health Care System to receive information about hospice services. We offer support to patients and families 24 hours a day, seven days a week. We will also contact the physician to ensure that the patient is eligible for the hospice benefit. If hospice is right for you, a hospice representative will meet with you and your family for admission to hospice. A plan of care will be developed by you, your physician, and the hospice interdisciplinary team that is specific to your needs.

Whether it’s in the comfort of their own residence, assisted living, or long-term care residence, our team of professionals and volunteers will provide them with the full range of hospice services they are entitled to.
