Who is eligible to receive hospice care?

 If you have Medicare you can use your home health care benefit if you meet all of the following conditions: 

Your doctor must order the home health care you are 'to receive.You must need one of Me following: 

intennittent skilled nursing care. or Ormeal/Speech Mae. or srroach-language Mara.. or comma. to nsai occupational therapy

You must be homebound, or normal, unaNe to leave you, home unasasted 

To be Homebound means that leaving home takes considerable and taxing effort

A person may leave home for medical treatment or short infrequent non-medical absences such as a religious service or a trip to the barber

The home health agency must be Medicare-certified

Hospice care in Western Pennsylvania

When curing a serious illness is no longer possible, many people choose to stop cu-rative treatment and accept palliative care. Palliative care means emphasis is placed on comfort and control of symptoms. It is not intended to prolong life. Hos-pice Care is provided primarily in the home, nursing home, or in an assisted living facility. 

The Hospice Team assists family members in caring for their loved one in the corn-fortable and familiar surroundings of their own home whenever possible. Anova Hospice not only focuses on the special needs of the patient, but also provides sup-port to their families.   • Do I qualify for home health services?
    In order to receive home care services,you must receive a written order for home health care by a doctor. Also, your care needs must be short term,not continuous, Finally, you must have a linited ability to leave your home or require considerable effort to do so.
  • How soon can i start services?
    Yes! To add a picture follow these simple steps: Enter App Settings Click the "Manage Questions" button Click on the question you would like to attach a picture to When editing your answer, click on the picture icon and then add an image from your library
  • Will you keep my doctor informed of my progress?
    Yes! Users can add video from YouTube or Vimeo with ease: Enter App Settings Click the "Manage Questions" button Click on the question you would like to attach a video to When editing your answer, click on the video icon and then paste the YouTube or Vimeo video URL That's it! A thumbnail of your video will appear in answer text box
  • What insurance do accept?How will I be billed?
    The FAQ title can be adjusted in the settings tab of the App Settings. You can also remove the title by unchecking its checkbox in the settings tab.


Palliative Care in Western Pennsylvania 


If you have a patient that is homebound or who you think may qualify for home care under any of the criteria listed, Anova Health Care System will complete an initial in-home physical assessment and evaluation. Our specialty-trained registered nurses will complete an extensive physical evaluation; obtain lab work, if needed and our staff will then relay all the necessary facts to assist the physician in making those important decisions.
